GymSAFE 15 Minute Injury Prevention Screen
Service Description
Our mission is to provide gymnastics professionals and athletes with the tools and knowledge to prevent gymnastics-related injuries and to promote the concept that physical and mental wellness are vital components of gymnastics excellence. The gymSAFE Screen is a standardized test that consists of gymnastics-specific tasks chosen to indicate movement dysfunctions linked to common gymnastics injuries. Message us to find out more! We offer Individual screens as well! ***The New Patient Paperwork must be filled out and sent back 24 hours prior to the scheduled appointment or we will cancel the appointment. You can find the paperwork on our website listed under "More" and we will email you a copy as well to the email address listed with booking. We also require a credit card to be placed on file or the patient cannot be seen per our policy. ***Please note: there is a premium rate applied for Holiday, emergency and weekend (Saturday/Sunday) appointments! This appointment type is not reimbursable by insurance and will therefore not be itemized into a Superbill. Please email admin@perfect10pt.org for specific pricing questions for this service. ATTN: This service is not covered my government issued insurance (ie. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill.
Cancellation Policy
To cancel or reschedule an appointment, please email, text, or call the office at least 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance, you will be charged the full scheduled service price as a cancellation fee. This policy applies regardless of reason. If you reschedule a canceled appointment within the same week, the cancellation fee will be waived. If you do not show up, you will be charged the full scheduled service price as a no-show fee. If required payment is not received prior to your appointment, you cannot be seen. This policy applies to the follow-up, recovery, screening, and performance services connected to it, including Gymnastics Performance Training Sessions with Ethan Valencia. Ethan's performance training is nonmedical and is not physical therapy. Service rates may vary by service or gym affiliation. Please email admin@perfect10pt.org for your specific rate BEFORE booking. Any applicable reservation fee is separate from the service fee and will be identified during booking. Required patient/client paperwork must be completed and returned by the deadline communicated by our office, or the appointment may be canceled. A valid credit card must be kept on file, and required payments must be made before the appointment. ATTN: Perfect 10.0 PT is a cash-based, fee-for-service practice and does not accept insurance. Gymnastics performance training is nonmedical and is not eligible for a PT superbill or medical insurance reimbursement. To ensure fairness and accommodate all patients' and clients' needs, Perfect 10.0 PT maintains a 24-hour advance booking and cancellation policy. Standard online bookings must be made at least 24 hours before the scheduled start time. -Emergency/Late Appointment Requests (<24 Hours): Requests made less than 24 hours before the appointment, including same-day requests, are subject to availability and approval. If accommodated, a single 50% surcharge (1.5 times the regular service rate) applies; same-day and late-request surcharges are NOT stacked. -Exception for Waitlist Patients: The late-request surcharge does not apply to patients contacted from the waitlist due to a same-day cancellation. ***Please note: A premium rate may apply to holiday and weekend (Saturday/Sunday) appointments.
