Telehealth Follow-Up PT Appointment

Service Description
If you are not wanting to leave the comfort of your own home-no problem! Let's set up a telehealth visit. I am using the Doxy.me website. Make sure you have a working camera on your computer or tablet, (cell phones do not have the proper picture quality) with lots of space to exercise. This appointment type is not reimbursable by insurance and will therefore not be itemized into a Superbill. ATTN: We are a cash-based, fee-for-service medical practice and do not accept insurance of any kind. This service is not covered my government issued insurance (ie. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill. This service is provided by a licensed physical therapist and is delivered as part of a structured physical therapy plan of care. Treatment interventions are selected based on individualized evaluation findings, clinical judgment, and patient-centered goals. Language such as ‘injury prevention’ and ‘performance improvement’ reflects risk reduction and functional enhancement strategies supported by clinical assessment, not guaranteed outcomes. If more than 30 days have elapsed from the patient's last visit, even by one day, a re-evaluation is required under Virginia physical therapy regulations to reassess the patient’s current status, progress, and plan of care before treatment continues.
Cancellation Policy
To cancel or reschedule an appointment, please email, text, or call the office at least 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance, you will be charged the full scheduled service price as a cancellation fee. This policy applies regardless of reason. If you reschedule a canceled appointment within the same week, the cancellation fee will be waived. If you do not show up, you will be charged the full scheduled service price as a no-show fee. If required payment is not received prior to your appointment, you cannot be seen. This policy applies to the follow-up, recovery, screening, and performance services connected to it, including Gymnastics Performance Training Sessions with Ethan Valencia. Ethan's performance training is nonmedical and is not physical therapy. Service rates may vary by service or gym affiliation. Please email admin@perfect10pt.org for your specific rate BEFORE booking. Any applicable reservation fee is separate from the service fee and will be identified during booking. Required patient/client paperwork must be completed and returned by the deadline communicated by our office, or the appointment may be canceled. A valid credit card must be kept on file, and required payments must be made before the appointment. ATTN: Perfect 10.0 PT is a cash-based, fee-for-service practice and does not accept insurance. Gymnastics performance training is nonmedical and is not eligible for a PT superbill or medical insurance reimbursement. To ensure fairness and accommodate all patients' and clients' needs, Perfect 10.0 PT maintains a 24-hour advance booking and cancellation policy. Standard online bookings must be made at least 24 hours before the scheduled start time. -Emergency/Late Appointment Requests (<24 Hours): Requests made less than 24 hours before the appointment, including same-day requests, are subject to availability and approval. If accommodated, a single 50% surcharge (1.5 times the regular service rate) applies; same-day and late-request surcharges are NOT stacked. -Exception for Waitlist Patients: The late-request surcharge does not apply to patients contacted from the waitlist due to a same-day cancellation. ***Please note: A premium rate may apply to holiday and weekend (Saturday/Sunday) appointments.
