Frequently Asked Questions
Perfect 10.0 Physical Therapy, is a unique, service-for-fee, cash based PT clinic. Here are some frequently asked questions!
FAQs
#1.
What is a cash based, fee-for-service, physical therapy clinic? Perfect 10.0 PT operates as a mobile, cash-based physical therapy clinic, offering direct care without the involvement of insurance companies. By eliminating third-party payers, patients gain access to personalized care without the inflated costs or restrictions typically imposed by insurance policies. A cash-based PT practice means we do not bill insurance directly. Instead, you pay for services at the time of your appointment. This model allows us to provide longer, more personalized sessions without the restrictions of insurance networks. Read More.
#2.
What is a superbill? A superbill is more than just a standard receipt. It’s a detailed summary of the services you received from a healthcare provider, including essential medical codes that your insurance company needs to process a reimbursement. Unlike traditional bills, which are sent directly to your insurer, superbills are provided to you, the patient, so you can file a claim yourself. Read More.
#3.
I already paid a $54 deposit fee, isn't this the cost of the Evaluation? No, a deposit fee is an upfront payment made to secure a service, appointment, or product before it’s provided.
It acts as a commitment from the client to move forward with the scheduled service while also protecting the provider’s time and resources.Think of it as a placeholder and once you pay the deposit, your spot is reserved specifically for you. The cost of the Evaluation is completely separate from the deposit fee and will be charged on the day of your appointment through a HIPAA compliant service provider. Read More.
#4.
You already have my credit card on file since I paid online, why do I have to give it again? The website (Wix) is not a HIPAA compliant website and therefore as a medical practice we cannot "save" your credit card on file when purchasing anything through the website. this is why we ask for it again to upload it into our billing platform that is HIPAA compliant for all future appointments.
#5.
What is your Cancellation, No Show and Pricing Policy? To cancel an appointment, or to reschedule please email, text or call the office 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance there will be a same service charge as a cancellation fee. If you reschedule a cancel appointment within the same week, the cancelation fee will be waived. If you do not show to your appointment there will be a same service charge for a no show fee. If payment is not received prior to appointment, you cannot be seen. There is a $54 non refundable deposit fee to reserve your booked Evaluation appointment time. AGAIN, this is non-refundable deposit and is not associated with the cost of the Initial Evaluation. Full payment for the Evaluation is due at the time of service and will be billed to the card we have on file. Pricing can be found in the New Patient Paperwork located on our Home Page under "More" pages 25-26 (Direct Link). Please email admin@perfect10pt.org for specific pricing questions for this service or your specific gym affiliation. Thank you!

How do I request a Superbill and what is your Policy? We provide a one time free courtesy Superbill at the time of discharge. You have to request this via our "booking services" tab. You can request as many superbills as you'd like, but there is a $67 fee associated with each request. Here is the direct link.

Why don't you accept insurance? By not contracting with insurance companies, we can:
-
Spend more one-on-one time with each patient
-
Treat based on your needs, not insurance limitations
-
Avoid unnecessary delays or denials in your care plan It’s about delivering better care, not more red tape.

What's the difference between the Evaluations? We offer 3 different types of Evaluations.
1. Initial Evaluation for Injury: this assessment is 60 minutes and primarily for Evaluation of pain or injuries, where Dr. Greene will examine, assess, diagnose and come up with a plan of care for your injured athlete. These injuries could range from acute to chronic. Read More.
2. Preventative Evaluation/Screen: this assessment is 60 minutes and is designed to help prevent gymnastics, dancing or cheerleading injuries by measuring range of motion, flexibility, mobility, strength, loading mechanics, landing mechanics and balance from head to toe. Read More.
3. Biomechanical Evaluation/Screen: this assessment is 90 minutes where Dr. Greene will run through several detailed biomechanical tests with your gymnast, dancer or cheerleader looking specifically at repulsion, take off, landing mechanics, loading, stability, and basic form/shapes. This is a great follow up assessment after the injury prevention screen to fine tune the gymnastics skills your gymnast needs. Read More.

Can I still use my insurance? Yes! While we don’t bill insurance directly, we can provide you with a superbill (a detailed receipt) that you can submit to your insurance provider. Many patients with out-of-network benefits receive reimbursement.
We recommend calling your insurance company to ask about out-of-network physical therapy coverage.
#10.
Can I use HSA or FSA card/funds? Absolutely. Most cash-based PT services are eligible for Health Savings Account (HSA) or Flexible Spending Account (FSA) reimbursement. You can use those cards just like a credit card at your visit or hold this card on file with sufficient funds pending.
#11.
Do I need a MDs referral to start PT? No, not in Virginia. We have Direct Access meaning you can walk right into a physical therapy office and be Evaluated. If we determine that you need further medical evaluation, we’ll refer you to the appropriate provider.
#12.
What's included in the session? Sessions are typically 60 minutes (depending on the session type) and 100% one-on-one with your PT.
You’ll receive:
-
A personalized assessment
-
Manual therapy and other modalities (if appropriate)
-
Targeted exercise and mobility work
-
Education and a plan for home/gym. There are no aides, techs, or waiting: just focused, individualized care.
#13.
How many session will I need? It depends on your condition and goals, but most cash-based clients need fewer visits than in-network PT because of the higher quality, focused care. We work together to find a plan that fits your lifestyle and recovery needs. We will have a plan in place after your Initial Evaluation with an estimated visits/week and an entire plan of care.
#14.

Pricing varies by gym affiliation, membership discounts and service type, but we offer transparent rates up front, just email us: admin@perfect10pt.org to get pricing information for your specific needs. No surprise bills. Seasonal packages or memberships may also be available for savings and ongoing support.
