​New Patient Onboarding Process

As part of our new patient onboarding process, we do require completed paperwork to be received and on file 24 hours prior to your scheduled appointment. Our practice operates as a mobile service, and we do not have the ability to photocopy or scan documents on site.

 

You are welcome to send your completed paperwork via email. Our email system on the practice side is secure and HIPAA-compliant. Please note that standard email transmission from personal devices or public Wi-Fi networks may not be fully secure.

 

If you prefer, paperwork may also be faxed to: 571-410-2230.

 

By choosing to submit your paperwork via email, you acknowledge and accept the potential risks associated with electronic transmission and confirm that this method is your preferred option.
