This is a one on one session with Dr. Ezara Greene, PT, DPT where she will examine, assess, diagnose and come up with a plan of care for your injured athlete. These injuries could range from acute to chronic. Set up an appointment today! Remember to wear loose clothing (leotards not required), and arrive 15 minutes early for additional paperwork. There is a $54 non refundable deposit to book this appointment to reserve your appointment time. Again, this is non-refundable. Full payment for the Evaluation is due at the time of service. ***There is a $8 additional gym fee and a premium rate for all Saturday Appointments! ATTN: This service is not covered my government issued insurance (ie. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill.
