Service Description

This screen is designed to help prevent gymnastics, dancing or cheerleading injuries by measuring range of motion, flexibility, mobility, strength, loading mechanics, landing mechanics and balance. There is a $54 non refundable deposit to book this appointment to reserve your appointment time. Again, this is non-refundable. Full payment for the Evaluation is due at the time of service. There is an $8 additional gym fee and a premium rate for all Saturday appointments. This appointment type is not reimbursable by insurance and will therefore not be itemized into a Superbill. ATTN: This service is not covered my government issued insurance (ie. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill.
