Service Description

These follow up appointments are one-on-one sessions with Dr. Ezara Greene, PT, DPT after you have received an Initial Evaluation for Injury. She will work on the deficits she found during the Evaluation and carry out your full Plan of Care to get you back to sport sooner, safer and in fewer visits. ***There is a $8 additional gym fee and a premium rate for all Saturday Appointments!

Cancellation Policy

To cancel an appointment, or to reschedule please email, text or call the office 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance there will be a same service charge as a cancellation fee. If you reschedule a cancel appointment within the same week, the cancelation fee will be waived. If you do not show to your appointment there will be a same service charge for a no show fee. If payment is not received prior to appointment, you cannot be seen. Thank you!
