Service Description

Prior to the Physical Therapy appointment all paperwork must me filled out with payment received 24 hours in advance. Screening will be performed before dry needling begins (if applicable)! Other Modalities include: cupping, Manual Therapy techniques, Massage and IASTM. Remember to stay hydrated throughout the day as well. Payment is due day of service in full. There is a 3.49% processing fee or $0.49 per transaction reflected in final payment. Please email admin@perfect10pt.org for specific pricing questions for this service or your specific gym affiliation. If payment is not received prior to the appointment, you cannot be seen. Thank you! ***Please note: there is a premium rate applied for Holiday, emergency and weekend (Saturday/Sunday) appointments! ATTN: We are a cash-based, fee-for-service medical practice and do not accept insurance of any kind. This service is not covered my government issued insurance (ie. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill. Please check with your insurance before using an HSA or FSA card on this service type, as it may not be covered. Perfect 10.0 Physical Therapy is also not responsible for the amount (if any) of reimbursement from your insurance company or any ICD 10/CPT codes that may not be covered by your specific insurance plan/policy. Please contact your insurance company directly for further information regarding your insurance policy. THIS SERVICE IS NON-FUNDABLE. Payment is due prior to the appointment booking. Cancellations of any kind will NOT be reimbursed. All rescheduling must be done on our app. Please download "Spaces by Wix" app. This is NOT an Evaluation. Dr. Greene will not use this time for an Evaluation, skilled PT rehab/exercises, assessing or Re-Evaluating old/new injuries. New injuries, or pain needs to be Evaluated PRIOR to treatment by booking an Initial Evaluation Appointment-Injury.
