Service Description
Recovery Sessions at Perfect 10.0 Physical Therapy support ongoing maintenance, performance, and recovery following completion of an Injury Evaluation or discharge from a plan of care. These sessions are for individuals who have been evaluated and are not currently presenting with a new or worsening condition requiring medical assessment. What to Expect: All services are provided by a licensed physical therapist within an established plan of care and are based on clinical judgment and ongoing assessment. Interventions may include: Manual therapy Dry needling Instrument-assisted soft tissue mobilization (Graston) Cupping Mobility and recovery-based techniques These services address recovery and performance needs and are not intended to evaluate or diagnose new injuries. Eligibility: Must have completed an Evaluation or plan of care. New or acute injuries require an Evaluation prior to treatment in accordance with the Virginia Physical Therapy Practice Act. Continued care is based on clinical appropriateness; re-evaluation may be required. Must be seen at least once per month to remain eligible. Payment & Policies: Payment required prior to appointment. 3.49% processing fee or $0.49 per transaction Non-refundable; cancellations not reimbursed. Reschedule via “Spaces by Wix” app. Additional rates apply for weekend, holiday, and emergency appointments. Contact: admin@perfect10pt.org Insurance: Cash-based practice; no insurance accepted. Compliance & Disclaimer: All services comply with the Virginia Physical Therapy Practice Act and are provided as skilled physical therapy within a plan of care based on medical necessity and clinical judgment. These sessions do not replace formal evaluation or rehabilitation when indicated. If symptoms change or fall outside PT scope, referral may be required. Participation carries inherent risk; no guarantees of outcomes are made. Important: Recovery Sessions are intended to support, not replace, comprehensive rehabilitation. Recovery Sessions are provided only after an Evaluation for Injury or discharge from care and are intended for maintenance and recovery. Not appropriate for new or worsening symptoms. Re-evaluation required for significant changes or if not seen within 30 days.
Cancellation Policy
To cancel an appointment, or to reschedule please email, text or call the office 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance there will be a same service charge as a cancellation fee. This policy applies regardless of reason. If you reschedule a cancel appointment within the same week, the cancelation fee will be waived. If you do not show to your appointment there will be a same service charge for a no show fee. If payment is not received prior to appointment, you cannot be seen. There is a $54 non refundable deposit fee to reserve your booked Evaluation appointment time. AGAIN, this is a non-refundable deposit and is NOT associated with the cost of the Initial Evaluation. Full payment for the Evaluation is due at the time of service and will be billed to the card we have on file. Pricing can be found in the New Patient Paperwork located on our Home Page under "More" pages 25-26. Please email admin@perfect10pt.org for specific pricing questions for this service or your specific gym affiliation. The New Patient Paperwork must be filled out and sent back 24 hours prior to the scheduled appointment or we will cancel the appointment. You can find the paperwork on our website listed under "More" and we will email you a copy as well to the email address listed with booking. We also require a credit card to be placed on file or the patient cannot be seen per our policy. ATTN: We are a cash-based, fee-for-service medical practice and do not accept insurance of any kind. This service is not covered my government issued insurance (i.e. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill. To ensure fairness and accommodate all patients’ needs, Perfect 10.0 Physical Therapy maintains a 24-hour booking and cancellation policy. This policy applies regardless of reason. -Same-Day Scheduling Fee: A 50% surcharge will be applied to appointments scheduled on the same day as the request. -Late Appointment Request Fee (<24 Hours): A 50% surcharge will be applied to any appointment requested less than 24 hours in advance. Exception for Waitlist Patients: Please note this policy does not apply to patients contacted from the waitlist due to a same-day cancellation. ***Please note: there is a premium rate applied for Holiday, emergency and weekend (Saturday/Sunday) appointments.
