Prior to the Physical Therapy appointment all paperwork must me filled out with payment received 24 hours in advance. Screening will be performed before dry needling begins (if applicable)! Other Modalities include: cupping, Manual Therapy techniques, Massage and IASTM. Remember to stay hydrated throughout the day as well. Payment is due at the time of booking the appointment time in full. There is a 3.49% processing fee or $0.49 per transaction associated with Wix reflected in final payment. If payment is not received at the time of booking the appointment, you cannot be seen. Thank you! ***All Saturday appointments are at a premium rate and an $8 additional gym fee will be applied. This item is NOT refundable by insurance and will therefore not be itemized into a Superbill. Please check with your insurance before using an HSA or FSA card on this service type, as it may not be covered. Perfect 10.0 Physical Therapy is also not responsible for the amount (if any) of reimbursement from your insurance company or any ICD 10/CPT codes that may not be covered by your specific insurance plan/policy. Please contact your insurance company directly for further information regarding your policy. THIS SERVICE IS NON-FUNDABLE. Payment is due at the time of appointment booking. Dr. Greene will not use this time for an Evaluation, skilled PT rehab/exercises, assessing or Re-Evaluating old/new injuries. New injuries, or pain needs to be Evaluated PRIOR to treatment.
