Service Description

This is a 10 minute scheduled phone call with Dr. Ezara Greene to discuss you or your child's plan of care, progress, return to sport protocol, answer any case related questions or concerns. This appointment type is not reimbursable by insurance and will therefore not be itemized into a Superbill. If the scheduled call goes past 10 minutes, client will be charged at a rate of $123.60/hour. Payment is due prior to appointment time in full. There is a 3.49% processing fee or $0.49 per transaction associated with Square reflected in final payment. Processing fees will not be refunded to your account if you are issued a full or partial refund. If payment is not received prior to appointment, you cannot be seen. There is a price of $62.22 no show fee and 24 hour notice cancellation policy. If you cancel less than 24 hours before your scheduled appointment you will be invoiced for $62.22. Thank you!
