Gymnastics Performance Assessment

Service Description
A comprehensive, one-on-one assessment designed to identify each gymnast’s individual performance needs, strengths, and goals. Your Sports Performance Coach will assess gymnastics-specific physical performance factors and use the findings to develop an individualized strength and conditioning program. Assessment may include: strength, flexibility, mobility, balance, coordination, power, movement quality, and exercise technique. The goal is to identify areas for development and create a personalized training plan designed to improve athletic performance, build physical capacity, and reduce injury risk. A Gymnastics Performance Assessment is required before beginning ongoing Gymnastics Performance Training. Important: This service is intended for healthy athletes seeking performance enhancement, strength and conditioning, and injury-risk reduction. It is a sports performance service and is not a physical therapy evaluation, medical treatment, or rehabilitation service. Athletes experiencing pain, a recent injury, swelling, loss of motion, or another suspected medical condition should schedule a Physical Therapy Injury Evaluation with Dr. Greene before participating. Performance services do not include diagnosis, medical advice, manual therapy, dry needling, joint mobilization, physical therapy treatment, or return-to-sport clearance. Medical questions involving pain, injury, rehabilitation, or return to sport will be referred to Dr. Greene or another appropriate healthcare provider. Athletes currently receiving physical therapy may participate when medically appropriate and consistent with their established plan of care. If signs or symptoms of injury or illness develop during a session, the Sports Performance Coach may pause training and recommend medical evaluation. Parents are welcome to discuss performance goals and training progress with the Sports Performance Coach before or after the session.
Cancellation Policy
To cancel an appointment, or to reschedule please email, text or call the office 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance there will be a same service charge as a cancellation fee. This policy applies regardless of reason. If you reschedule a cancel appointment within the same week, the cancelation fee will be waived. If you do not show to your appointment there will be a same service charge for a no show fee. If payment is not received prior to appointment, you cannot be seen. There is a $54 non refundable deposit fee to reserve your booked Evaluation appointment time. AGAIN, this is a non-refundable deposit and is NOT associated with the cost of the Initial Evaluation. Full payment for the Evaluation is due at the time of service and will be billed to the card we have on file. Pricing can be found in the New Patient Paperwork located on our Home Page under "More" pages 25-26. Please email admin@perfect10pt.org for specific pricing questions for this service or your specific gym affiliation. The New Patient Paperwork must be filled out and sent back 24 hours prior to the scheduled appointment or we will cancel the appointment. You can find the paperwork on our website listed under "More" and we will email you a copy as well to the email address listed with booking. We also require a credit card to be placed on file or the patient cannot be seen per our policy. ATTN: We are a cash-based, fee-for-service medical practice and do not accept insurance of any kind. This service is not covered my government issued insurance (i.e. Medicaid, Medicare Part A and B and Tricare) and therefore is not reimbursable and will not be itemized into a Super Bill. To ensure fairness and accommodate all patients’ needs, Perfect 10.0 Physical Therapy maintains a 24-hour booking and cancellation policy. This policy applies regardless of reason. -Same-Day Scheduling Fee: A 50% surcharge will be applied to appointments scheduled on the same day as the request. -Late Appointment Request Fee (<24 Hours): A 50% surcharge will be applied to any appointment requested less than 24 hours in advance. Exception for Waitlist Patients: Please note this policy does not apply to patients contacted from the waitlist due to a same-day cancellation. ***Please note: there is a premium rate applied for Holiday, emergency and weekend (Saturday/Sunday) appointments.
