Service Description

Looking for some direction or advice on a problem area? Sign up for a PT Injury Quick screen and let us help direct you to the right health care provider/facility. This is a screening process for an acute or chronic injury triage. Dr. Greene will take a subjective and then triage to the appropriate service for you or your athletes need. ***There is a $8 additional gym fee and a premium rate for all Saturday Appointments!

Cancellation Policy

To cancel an appointment, or to reschedule please email, text or call the office 24 hours PRIOR to your appointment. If you cancel less than 24 hours in advance there will be a same service charge as a cancellation fee. If you reschedule a cancel appointment within the same week, the cancelation fee will be waived. If you do not show to your appointment there will be a same service charge for a no show fee. If payment is not received prior to appointment, you cannot be seen. Thank you!
